The Home Care Package (HCP) is a government-subsidized program in Australia designed to support older individuals in living independently at home for as long as possible. It provides a tailored range of services based on an individual’s needs, preferences, and goals. The program is part of the Australian Government's aged care system.

Key Features of the Home Care Package:

  1. Levels of Support:
    There are four levels of Home Care Packages to match the complexity of care required:

    • Level 1: Basic care needs
    • Level 2: Low-level care needs
    • Level 3: Intermediate care needs
    • Level 4: High-level care needs
  2. Services Provided:
    Services are flexible and can include:

    • Personal care: Bathing, dressing, grooming
    • Health services: Nursing, allied health, and therapy
    • Domestic assistance: Cleaning, gardening, meal preparation
    • Transport: Assistance getting to appointments or shopping
    • Social support: Help staying connected to the community
  3. Funding:

    • The Australian Government provides funding for Home Care Packages, but recipients are expected to contribute based on their income.
    • A care plan is developed with the funding allocation to meet specific needs.
  4. Eligibility:
    To qualify, individuals need to:

    • Be 65 years or older (or 50+ for Aboriginal or Torres Strait Islander people)
    • Undergo an ACAT (Aged Care Assessment Team) assessment to determine their care level and eligibility.
  5. Providers:

    • The package is delivered by approved home care providers, who work with recipients to manage and coordinate services.
    • Recipients have the flexibility to choose their provider and switch if needed.
  6. Self-Managed or Provider-Managed:
    You can either let your provider manage your package or take more control and self-manage it, depending on your preference.
